SCHOFIELD, United States District Judge: Plaintiff brings this pro se action, for which the filing fees have been paid, alleging that Defendants discriminated against him on the basis of his race and age, in violation of the Age Discrimination in Employment Act of 1967 (“ADEA”) and 42 U.S.C. § 1981. A. Defendants JF Restaurants, John Fraser, and Amy Racine The Clerk of Court is directed to issue summonses as to Defendants JF Restaurants, John Fraser, and Amy Racine. Plaintiff is directed to serve the summons and complaint on each defendant within 90 days of the issuance of the summons. If within those 90 days, Plaintiff has not either served Defendants or requested an extension of time to do so, the Court may dismiss the claims against Defendants under Rules 4 and 41 of the Federal Rules of Civil Procedure for failure to prosecute. B. Defendants Human Resources Managers Ben and Mariana Under Valentin v. Dinkins, a pro se litigant is entitled to assistance from the district court in identifying a defendant. 121 F.3d 72, 76 (2d Cir. 1997). In the complaint, Plaintiff supplies sufficient information to permit JF Restaurants to identify Ben and Mariana, whom Plaintiff claims are human resources managers. It is therefore ordered that JF Restaurants, the employer of Ben and Mariana, must ascertain the identities of these defendants and provide their full names and the addresses where they may be served. JF Restaurants must provide this information to Plaintiff and the Court within sixty days of the date of this order. Within thirty days of receiving this information, Plaintiff must file an amended complaint providing the full names of Ben and Mariana. The amended complaint will replace, not supplement, the original complaint. An amended complaint form that Plaintiff should complete is attached to this order. Once Plaintiff has filed an amended complaint, the Court will screen the amended complaint, and issue an order directing the Clerk’s Office to issue summonses for the newly named defendants, and directing Plaintiff to serve them.
